Important Information Regarding Obesity Management Medication

If you currently take an Obesity Management Medication, you must transition your weight care to Form Health by January 31, 2027.

Starting February 1, 2027, Obesity Management Medications, such as GLP-1s, must be prescribed by a Form Health physician. This change does not affect you if you are taking an Obesity Management Medication to treat type 2 diabetes.

Form Health offers personalized, medically guided care that can help you take the next step in your health journey with confidence.

If you currently take an Obesity Management Medication, you will receive communications on how to transition your weight care to Form Health by the January 31, 2027, deadline.

Employees and dependents aged 18+ who meet program criteria and are enrolled in an HII Anthem health plan must use the Form Health program for Obesity Management medications. There is no member cost share for Form Health services, but a $50 co-pay will apply to each prescription.

Say Hello to Frank—SmartPath’s Pilot Program Launching Soon

Your HII financial benefit SmartPath is launching a pilot program for Frank, a new tool designed to help you stay on top of your finances with less stress. Frank keeps an eye out for things like rising credit utilization, unused cash sitting in accounts, new scam alerts, and possible gaps in your auto insurance—then gives you a quick heads-up when something needs attention.

During this pilot, a select group of HII employees will be invited to give Frank a try and share feedback. If you’re chosen to participate, you’ll receive an email from SmartPath with all the details on how to get started.

As part of the pilot, you’ll also get a full year of Frank for just $12 (normally $180). It’s an easy, low-cost way to test out a tool designed to make managing your money simpler and less stressful.

Meet Leap—Your New HII Benefit for Infusion Care

With Leap, you can receive infusion treatments from the comfort of home from a licensed infusion nurse. Schedule treatments when it’s most convenient for you (including evenings and weekends), and a dedicated Leap Care Guide will handle the details so you have less to worry about.

Leap supports infusion treatments for many conditions, including multiple sclerosis, lupus, chronic inflammatory disorders, Crohn’s disease, chronic kidney disease, bleeding disorders, hematology, immunoglobulins, and some oncology treatments.

Breast Cancer Awareness Month: Know Your Risk, Get Screened

Every two minutes, someone in the U.S. is diagnosed with breast cancer — but most breast cancers are found early through routine screenings, when treatment is most successful. This October, take a moment to understand your risk and stay on top of your health.

Your HII benefit providers Color Health and the HII Family Health Centers make breast‑cancer screening simple. With both, you can:

  • Learn your personal risk and get a tailored screening plan
  • Easily schedule a mammogram at a nearby, in‑network location
  • Get guidance and answers from expert physicians
  • Access screening resources for other common cancers

The American Cancer Society recommends annual mammograms starting at age 45 for women at average risk, with the option to begin at age 40. Women at higher risk — due to factors like family history, genetic mutations, or dense breast tissue — typically begin annual mammograms and breast MRIs at age 30.

No matter your age or risk level, talking with a health‑care provider can help you determine the right screening plan for you.

THE KNOW YOUR NUMBERS PROGRAM

Get Your Biometric Health Screening at Your Worksite

When you understand what’s going on with your overall health, you’re better equipped to make smart choices that prevent disease and help you feel your best. That’s exactly what Know Your Numbers is all about. This quick screening gives you a snapshot of key health indicators—like blood pressure, cholesterol, and A1C—so you can spot potential risks early and make informed decisions about your health.

And while you’re there, you can take care of two things at once: flu shots are available at Know Your Numbers screening events, making it easy to knock out both your biometric screening and your flu vaccine in a single visit.

Why Participate? 
Beyond the personal health benefits, completing the program could help you save $600 annually on your HII Anthem medical insurance premium. Employees who participate but do not qualify for the premium discount will be entered into a quarterly wellness prize drawing. 

Program Dates and Requirements 
The program runs each year from July 1 through March 31. To earn the $600 premium savings beginning July 1, eligible employees must complete both program requirements by March 31: 

  1. Health Check Survey 
    Complete the online survey through QuadMed MyChart.
  2. Biometric Health Screening 
    Get screened at an HII Family Health Center, your personal healthcare provider, a Quest Patient Service Center*, or at designated onsite events — including those starting this month.
