Know Your Numbers: Put Know Your Numbers to Work For You

After you’ve completed your biometric health screening and health check survey, it’s time to put that knowledge to work — especially if you need to keep your blood pressure and blood sugar numbers in check.

Tips + Resources from the HII Family Center

  • SET A GOAL. Partner with your health care provider to set a target for your blood pressure and/or blood sugar numbers.
  • KEEP TRACK. Monitor your blood pressure and/or blood sugar at home. Your care provider can show you how to get an accurate reading.
  • REMEMBER TO SHARE. With MyChart, it’s easy to send your results directly to your care provider between appointments. Your provider can use this information to adjust your medications, which can help you reach your goal.
  • FOLLOW DIRECTIONS. Take your medications exactly as directed by your care provider. Not sure how to take them or having side effects? Talk with your provider or pharmacist.
  • KNOW YOUR BENEFITS. Make sure you take advantage of all the resources offered by your HII benefits — from HII Family Health Center nutritionists and wellness coaches, to BeWell for Life programs and Vida.
  • STAY SMART. Learn all you can about your health condition so that you (and your health care provider) can make educated decisions and take control of your health.
